Kidnapped John Hallucinates Fic

I've been searching for a fic I read quite a long time ago. It's set in the BBC Sherlock universe and in it John gets kidnapped by Moriarty during The Great Game Episode then tortured and stuck in a car with a bomb strapped on him, a cellphone, and access to surveillence on Sherlock. During this he starts hallucinating that Sherlock is sitting next to him during the whole thing. I think it is part of a series.
