Canon Watson h/c Fic

Watson had just returned from Afghanistan and his medical association tells him he's unfit to perform surgery and has to retire his surgical medical career to be just an ordinary house doctor.  He gets very depressed over this and doesn't sleep or eat very well.   Holmes is on a case and is too busy to notice Watson's unhappiness.  He walks into town and witnesses a train derailment and ends up working for hours at the scene, treating the injured.  When he comes home, Holmes is on the steps waiting anxiously for him, knowing he was near the place where the train derailment happened and wondered if he got hurt.  Watson has worked too long and hard at the scene and becomes sick later with fever and Holmes and a doctor friend of Watson's tend to his recovery.  When he gets better, Holmes tells him he's a fine doctor even if he can't do surgery anymore and he's been receiving letters of thanks from the people he had treated at the train derailment. 

Does anyone know this fic?  I remember reading the author's other works, and s/he does a very good job of writing a flawed book!watson.  so any help will be great!
