Let's see. Most of what I have is BBC. I'll note any exceptions.
Note - I'm listing all the various things I have tagged relating to illness, although they may not play up the vulnerability in quite the way you have in mind.
John, ill Consumed by ancalime8301 (ACD verse) Warning: tragic. Watson learns that he has tuberculosis after Holmes' 'death' at Reichenbach. He keeps a stiff upper lip about it. (This qualifies under respiration problems for your request, but it is not really a manageable illness in this 'verse.)
Underneath the Black Night by miss_variety (ACD verse) Warning: tragic. Sounds similar to the above if summarized, but in this case Watson keeps even more quiet about his illness.
Sherlock, ill Adjusting by mogwai_do This one is unusual - it's an AU in which Sherlock's illness is vampirism. Being Sherlock, he gets distracted and doesn't take care of himself properly.
Low by lindor1306 Sherlock has diabetes. John would've preferred to find out about this some other way than by Sherlock collapsing at a crime scene. The Yard team is treated sympathetically in this one.
Wrong Order by dayja Sherlock is allergic to mint. Some of the Yard team try to play a practical joke with this, with very serious results.
