Found! Kindnaped!John

I’m looking for a BBC Sherlock/John story. It is basically the events of The Great Game but John was kidnapped and Sherlock has to figure out all the problems Moriarty has setup to get him back. The one thing I remember clearly is when John (In the part where the old lady was) speaks without Moriarty’s prompting and gets annoyed at Sherlock because he was playing with his life, Sherlock freaks out and then John says, “Oh Sherlock, what have you done?” and then the bomb goes off. (But it is revealed later that John was never in the explosion. Does anyone know what story I’m talking about?

Edit 1: I think, I think, John had an imaginary Sherlock because Moriarty was torturing him.
