Doctor Who/Sherlock Crossover

I know there have been a few brilliant crossovers with Doctor Who and Sherlock, but there's one specific one that I thought I'd bookmarked but clearly... hadn't. It was set at the end of season three of new!Who, in the year between The Sound of Drums and The Last of the Time Lords. John is a doctor in some... labour camp or other, and, as medical staff, has a license to travel. He's sent somewhere else (vague, I know, sorry), where he meets Sherlock. It turns out that Sherlock's working with the resistance, and he utilises John's freedom to help his cause. They separate, but meet up again, by which time I think John's also started working against The Master and co. In the end, Sherlock is killed and John injured by the Toclophane (which is probably spelled wrong, but oh well), and John is the last connection to Tom Milligan (before he meets Martha at the beginning of The Last of the Time Lords) before dying of an infected leg wound.

Sorry it's a bit vague in places, but - ring any bells? I've been re-watching Doctor Who, and I'd love to re-read the fic and establish it as head!canon before watching The Last of the Time Lords.
